Job Summary : Provides and directs the provision of nursing care in the home, based on the agency policies and procedures through the competent application of the nursing process after normal business hours.
Plans and implements the total palliative care of the patient in support of the prescribed medical team plan. Assists with and / or reinforces patient teaching.
Provides direct nursing care, with special emphasis on control of pain and other symptoms of the hospice patient. Provides nursing services to patient / family utilizing the hospice concept.
Participates in staff meetings and team conferences. Reports to Clinical Practice Supervisor.
Minimum Requirements : Graduate of approved school of professional nursing.
Minimum of one year of nursing experience preferred.
Must be able to travel to patient care assignments over multi-county area. Access to telephone. Must be flexible to meet patient care needs.
Must be able to work on-call.
Preferred Qualifications : Hospice certification preferred but will accept working toward certification.
Licensure and Certifications : Current Pennsylvania State RN license.
Must possess and maintain current driver's license. Must provide same upon request. Must possess and maintain personal automobile insurance at PA State minimums.
Must provide proof upon request.
