High school diploma or equivalent is required
The Community Health Worker (CHW) provides community support for patients within their respective primary care office to help to improve health outcomes for patients with complex medical, social or behavioral needs.
The CHW will provide care coordination services to address health and social needs of identified patients within respective primary care offices.
Together with the Primary Care Provider, Case Manager and Health Coach, the Community Health Worker will assist in care plan execution, develop care management strategies for enrolled patients and provide linkages for the various health and social needs of patients.
When needed, the CHW will also engage the assistance of the Super Utilizer team.The CHW will work in the field in a variety of Delaware County settings, including patient homes, medical offices, homeless shelters and the ED / inpatient of Crozer Keystone Health Systems (CKHS) hospitals.
Education and Training :
High School Diploma or equivalent required
Medical Assistant program certification preferred
Experience and Knowledge :
Current or former resident of Delaware County preferred
Experience in community / outpatient setting preferred
Requires the ability to travel to multiple office locations; valid driver's license and automobile that is insured
Demonstrates ability to carefully set priorities, meet deadlines, and schedule time efficiently
Ability to accurately type and effectively operate a computer and use a variety of common software programs
Ability to follow-up and follow-through with strong attention to detail
Ability to work well and maintain professionalism under occasional times of stress and pressure
Demonstrate ability to follow oral and written instructions
Demonstrate ability to work with other people through a cooperative effort
Possess strong interpersonal skills as demonstrated by compassionate, courteous, cordial, cooperative, and professional interaction with diverse groups of co-
workers, external business partners and the community
